Lakeland Regional Boys Lacrosse - 2020 Player Registration

Lakeland Lacrosse Association 2020 Season Player Registration Form

Basic Player Registration Fee: $150 ($100 additional family member)
This is a mandatory registration fee that each high school boy or girl player pays to be registered as a member of LLA. Players must be fully paid members to receive the Player Pack and players admission to the team dinner.

NOTE: New players are required to buy their own equipment (LLA approved only).

2020 Player Packs will include: Sweatshirt | Sweatpants | Shooter Shirt | Shorts | Pinnie
√ NEW FOR 2020: Hoodies and Sweatpants are considered part of the player uniform and must be worn to every game. 
√ Pinnie must be worn to all practices. 

Please bring Payment to the Parent Informational Meeting on February 3, 2020.
This meeting will take place in the LRHS Cafeteria at 7PM. 

Parent and Player Liability Release

Lacrosse is a full contact sport in which participants can be injured. By signing below, you agree to release and hold harmless the Lakeland Lacrosse Association (“LLA”) and its directors, officers, representatives, coaches, facilities (including games, practices and clinics) sponsored or sanctioned by LLA.

Photographic Release

By signing this form, the parent/guardian gives permission and waives the right to any type of compensation for their child to be photographed or videotaped at any or all activities sponsored by LLA. Pictures or photos may be used for publicity by LLA on their web site, email, flyers, projects, etc.
