Spring Sports Physical

  Physicals for spring sports will be held next Monday, 1/11/10 from 5 – 7 PM in the Home Living room (5104). Parents please enter through the doors by the 5100 hall (upon entering the building, they will take a right towards the Home Living room).  

  Here is what each student will need to have with them to get a physical:

• The  Resurgens Liability Form  filled out and signed by a parent/guardian
• The  GHSA Preparticipation Physical Evaluation  form filled out (first page only)
• $20 (checks made payable to AHS)
