Physicals

In preparation for participation in athletics for the 2016-2017 school year, please consider taking advantage of the free Pre-Participation Physicals offered by OhioHealth during the month of May. These physicals will be good for the entire school year next year. Beginning May 1, you will be able to log into your FinalForms account and print the physical forms to take to the sites. You will also be able to register for next years sports and Marching Band. Below is a list of dates and locations.
