Things to Remember
- Include the salesman's name on each form to ensure proper credit for the sale.
- We need complete customer information, including name, address, phone number, and subdivision (if applicable).
- This year's form includes DELIVERY INSTRUCTIONS. Please include these by using the check-off boxes or by writing in specific instructions. This saves the time required to call each customer for instructions.
