2015 Men’s Lacrosse Registration
Step #1: Fill out the online form below.
Step #2: Fill out the School/County athletic forms in the AHS Athlete Registration Packet.
Step #3: Parents and Athletes must read the Player Handbook. Parents and Athletes must sign/date and print out the contract on pages 28-29.
Step #4: Turn in the forms (Step#2) and signed/printed contract (Step#3) to Coach Jacobs, Room #4310 by January 12.
Step #5: Pay dues at the February 3rd Spring Kick-off.

2015 Men’s Lacrosse Important Dates
December 1st– Registration Opens. All completed forms (See Parent Checklist) should be turned into Coach Jacob’s room 4310
January 12th – Last date to turn in Registration Forms. Prospective players will not be allowed to tryout if forms have not been submitted.
January 26-28th – Tryouts time 6:00-8:00pm, AHS Stadium Turf Field
Final Rosters Announced by Wed. 1/28
January 28th – Teams Announced
First Practices, Thurs 4-6:00 practice field. Friday 4-6:00 in stadium.
First Practices, Thurs 6-8:00 stadium. Friday 4-6:00 in stadium.
February 3th – Spring Season Kick-off: Player/Parent Meet and Greet, Potluck
6:00 at AHS Cafeteria
- Player Registration Payment Due
- Parent Volunteer Form Due
- Players will not receive their uniforms and player packs if payment and volunteer forms are not turned in.
