Resurgens will be at Alpharetta High School

Hello Everyone,

Resurgens will be at Alpharetta High School to conduct physicals on Monday, April 20^th at 5:00 PM in the gym (health rooms located by the weight room). The cost is $20. These physicals will be valid through May 2010 (the entire next school year).  Press more [+] for details.

*Physical participants need to come with the following:

1. The physical form from the GHSA website (first page filled out by the parent/guardian): http://www.ghsa.net/files/documents/forms/PhysicalEvaluation.pdf
2. The attached Resurgens liability form filled out by the parent/guardian
3. Cash or check for $20 
4. Resurgens Liability Release Form
